How to Register Your Oklahoma Medical Marijuana License
1. Get certified with Ozark MMJ Cards.
First things first — Getting your certification is only half the work, but you won’t be able to register for your license without it. Fill out our Oklahoma Medical Marijuana Application Form to get approved. Once you complete the process, meet with the doctor, and obtain the physician’s certification from us, you are ready to register for your Oklahoma Medical Marijuana License with the state of Oklahoma via the Oklahoma Medical Marijuana Authority, (OMMA).
2. Get your documents ready.
The documents you’ll need on hand are:
- Physician’s Recommendation Form (certification provided by us)
- Proof of Medicaid/Medicare/Disabled Veteran Status (optional)
- Digital Photo
- Proof of Identity
- Proof of Residency
- Payment via Credit Card ($100 application fee; $20 if medicaid/medicare/disabled veteran)
- Create an Account (Keep your credentials in a safe place. You will need access to your account in order to renew later).

3. How to Register.
Once you’ve got your documents sorted, you’re ready to register with OMMA. Visit the OMMA application page to log in, follow the steps on the screen, and submit your application.
OMMA will then review your application for completion and eligibility. You can expect an approval letter with your license with your Oklahoma Medical Marijuana License in the mail within 14-business days of the application submission date.
If an application is deemed incomplete, you’ll be notified via email to log back into your account and make corrections to your application.
